If you're responsible for managing the estate of someone who requires special care in their home, MSN HomeCare offers a variety of family-centered care options. Whether it is a medically fragile long-term care case or an individual just needing a bit of extra support or community resources, Medical Staffing Network has qualified care management specialists (CMS) to assist clients and families in need of support or assistance. Medical Staffing Network can manage and provide the necessary tools and resources, as well as the clinical and non-clinical support for yourself, your client and their families. Given the complexity of the healthcare programs, treatment plans, social services and community resources, a care management professional is a smart solution to positive out-comes and patient/family advocacy.

If you are seeking provider services, our trained and experienced home health care professionals include both skilled and non-skilled pediatric and adult caregivers. Homecare and our Care Management divisions provide a wide array of healthcare professionals including;
  • RN's
  • LVN's
  • CNA's /HHA's/Sitters

RN Case Managers who can coordinate additional provider services such as LMSW's, OT's, PT's, and ST's.

Our nurses are fully licensed and all our caregivers receive clinical support and supervision 24 hours a day, 7 days a week by the Branch office D.O.N. MSN staff must meet very stringent hiring criteria equivalent to the Joint Commission's, which is the gold standard for hospital/facility nurse staffing. Our professionals are thoroughly familiar with community and national health resources and funding programs and offer both home care and case management perspectives.

If only Care Management services are needed, arrangements are made with the referral source for definition of the service needs and then a clinical assessment of the client is done by the agency RN Case Manager in congress with recommendations from hospital case manager, physician/care team, and family members when appropriate. Care Management services include a variety of supports /services including assistance with the scheduling of or accompanying to Dr. appointments ,patient advocacy, assessments for changing levels of care, the arranging of other support services or financial assistance such as Medicare benefits, OT,PT, and Speech therapies and special monitoring in times of grief or post surgically. The RN Case Manger providing Care Management services is also available 24 hrs on-call.

To make a referral, contact your local MSN HomeCare office. Upon contact, our RN Case Manager will assist you by scheduling and coordinating an in-home or facility assessment to determine the needs of your family member, yourself, or your client, to develop a plan of care and plan to begin services. Once care begins we can make doctor's appointments, coordinate activities or services and provide clinical/psychosocial supervision

Case Managers
MSN HomeCare offers a variety of family centered care options. Our home health care professionals include both skilled and non-skilled pediatric and adult caregivers. They are experienced with both pediatric and adult patients. Our nurses are fully licensed and all our care givers receive clinical support and supervision 24 hours a day, 7 days a week.

If you are a Facility Case Manager looking for provider options during the discharge process, MSN HomeCare offers two divisions:
  • Provider Services
  • Individual on-going Case Management

Your client may be interested in medical or non-medical providers in the home and request a list of recommended companies during the discharge planning process. If so, MSN HomeCare will work with you and your client to determine the most appropriate caregiver based on the needs of the patient while working closely with you, the physician/care team, and even family members to determine the types of support needed for home services. Once the level of need is determined, a MSN HomeCare RN will provide an in-home assessment as well as a clinical assessment to further support definition of services in the home. The RN will then arrange for the appropriate staff to begin care as soon as possible for both client and agency. If skilled care continues for a period of time, an assessment/supervisory visit will be repeated every 60 days by a RN with RN on-call access 24hrs a day while MSN HomeCare provides services in the home. This type of referral is made simply by contacting your local MSN HomeCare office and you will be connected to our intake department or RN Case Manager who can discuss the details of the discharge and begin the process by with a Fax transmittal of the appropriate documents. The agency RN Case Manager is available to discuss any questions, concerns initially and an appointment for the home assessment is scheduled. In addition, families sometimes request an in-hospital introduction of the agency and if so, that can be arranged as well.

MSN Home Care offers flexible options, from four hours (whenever the patient needs them to complete) to round the clock, 24-hour private duty care. MSN Home Care also offers high-tech services, with a respiratory therapist on staff.
